WEST JORDAN, Utah – An Orem man died after an industrial accident in West Jordan Friday morning.
Officers said 37-year-old Alfonso David-Grajeda was working on a steel structure near at SME Steel, 5801 W. Wells Park Rd. at about 6:30 a.m. when two 40-foot steel tubes fell on him and crushed his head and neck.
The Utah Occupational Safety and Health Division (UOSH) will be investigating this incident.
