SALT LAKE CITY — Multiple agencies responded to a serious crash at State Road 201 and 3200 west around 5:15 p.m. Saturday evening.

Lt. Victor Siebeneck with the Salt Lake City Police Department says a semi-truck collided head-on with a minivan.

The investigation as to how the vehicles collided is still underway, but Lt. Siebeneck said a red minivan was southbound on 3200 west driving toward westbound 201 and the truck was northbound on 3200 west and attempting to get onto westbound 201.

The van took the brunt of the damage.

“In the van we have several occupants, two of whom had to be extricated, one was ejected, that was a juvenile who was transported up to Primary Childrens Hospital,” Lt. Siebeneck said.

All of the injured are said to be in stable condition.

The truck involved in the crash is part of a driving school.  A student was driving and the instructor was a passenger.  Neither was hurt.
